Medical consent for GLP-1 therapy

By becoming a MediFit Clinic patient you confirm that you have read and understood the following information and consent to GLP-1 receptor agonist therapy where prescribed.

What is being prescribed

Compounded semaglutide or compounded tirzepatide — both GLP-1 receptor agonists — dispensed by a SAHPRA-licensed compounding pharmacy under prescription by Dr. Raythaan Addinall.

Known side effects

  • Nausea, vomiting, constipation, or diarrhoea (common, usually mild)
  • Fatigue, headache, dizziness (common, usually transient)
  • Reflux, abdominal discomfort (common)
  • Gallbladder disease and pancreatitis (uncommon, serious — seek urgent care)
  • Hypoglycaemia, especially if used with insulin or sulphonylureas (uncommon)
  • Allergic reaction (rare)

Known contraindications

  • Personal or family history of medullary thyroid carcinoma or MEN-2
  • Pregnancy, breastfeeding, or active attempts to conceive
  • Severe gastrointestinal disease (e.g. gastroparesis)
  • History of pancreatitis

Off-label use

Some prescribing — including the use of tirzepatide for weight loss — may be off-label in South Africa. Off-label prescribing is permitted under HPCSA guidelines when supported by clinical evidence and patient consent. Dr. Ray will explain any off-label use during your consultation.

Compounded medication

Compounded medications are prepared in SAHPRA-licensed compounding pharmacies and are dispensed for a named patient under prescription. They contain the same active molecules as branded equivalents but are not branded products and are not assessed by SAHPRA as a registered medicine.

Your acknowledgement

You confirm that you have disclosed all relevant medical history, are not pregnant or breastfeeding, and have had an opportunity to ask Dr. Ray any questions about the protocol. You agree to stop treatment and seek urgent care if you experience severe symptoms such as ongoing vomiting, severe abdominal pain, or signs of allergic reaction.
